• PRIVACY CONFIDENTIALITY SECURITY AGREEMENT – 26.01.08
• Student Declaration – Work Experience – 38.04.04
• Staff and Volunteer Image Consent Form
• Hand Hygiene Workbook

Items to be uploaded PRIOR to your placement commencing are:

• Evidence of COVID-19 Vaccination status
• Hand Hygiene Workbook
• Child Safety Standards
• Privacy and Confidentiality Agreement
• Student Declaration
• Staff and Volunteer Image Consent

All six documents must be finalised and uploaded at the same time.
