An opportunity exists for a full time Care Coordinator in our Community Nursing team.

Community Nursing is a part of the broader Community Services Department based in Ararat. We provide community nursing across the catchment area of Ararat Rural City. Community nursing includes palliative care, district nursing, hospital in the home, hospital admission risk program (HARP), home care packages and specialist nursing programs. Each program area aims to provide awareness and education which enables community members to have a better understanding of their own health issues and thus take greater responsibility for improving their own health outcomes.
Community Nursing Care Coordinators provide effective management and coordination of a clients home based nursing care including District Nursing, Hospital in the Home and Palliative Care. Coordinators are responsible for assessing program eligibility, service coordination, client assessment and review. Care Coordinators are the primary clinical contact person for all relevant stakeholders and oversee community nursing staff in the delivery of care.
Essential criteria include current registration with AHPRA as a Registered Nurse, a demonstrated ability to assess, plan and coordinate care for clients with complex medical, psychological and chronic illness and knowledge of community nursing funding programs. A post graduate qualification in management or interest in post graduate study will be considered favourably.
Appointment is subject to immunisation clearance and current and satisfactory Employment Checks (Police and Working With Children Checks)
