We are currently looking for an Enrolled Nurse to fill the role of Healthy@Home Care Coordinator. This is a part time position, 28 hours per week.

East Grampians Health Service (EGHS) Community Services aim to meet the needs of the community by offering a vast array of services to the community in Ararat and surrounding districts. We offer outpatient, outreach and domiciliary services. Our services include Allied Health, Dental, Community Nursing, Women’s Health, Home Care Packages, Home Support Services and Patricia Hinchey Centre.  These services support other clinical areas including our acute and residential aged care consumers.  EGHS@Home includes our Home Support program, Healthy@Home (telehealth program), Support at Home and Carers Support program.

What We Offer

About The Role

Healthy@Home is a step-down service from the Hospital Admission Risk Program (HARP). Telehealth Assistants call participants, when a risk to a consumer’s home or health status is flagged, this is escalated to the Care Coordinator who acts accordingly, thus reducing the risk of an unplanned hospital admission.

The Care Coordinator establishes trusted relationships with participants and on the participant’s behalf, bridges boundaries between services. The Care Coordinator will support clinical management within scope of practice, promote self-management and support their participants and their carer’s to navigate and access relevant services. The role will involve establishing effective and collaborative relationships with the HARP team and other service providers. As the participants needs are diverse and often complex, there will be a strong focus on service development and participant centered care.

About You

  • Essential criteria include current registration with AHPRA as an Enrolled Nurse
  • Demonstrated ability to assess, plan and coordinate care for clients with complex medical, psychological and chronic illness
  • Ability to engage vulnerable or marginalised people to address their complex health care needs

Appointment is subject to immunisation clearance and current and satisfactory Employment Checks (Police and Working With Children Checks and NDIS Worker Screening Check).
