We are seeking suitably experienced, dynamic and innovative Registered Nurses to work in our Community Nursing Care Coordination Team.
This is a permanent position, up to full time hours.

Community Nursing is a part of the broader Community Services Department based in Ararat. We provide community nursing across the catchment area of Ararat Rural City. Community nursing includes palliative care, district nursing, hospital in the home, hospital admission risk program (HARP), home care packages and specialist nursing programs. Each program area aims to provide awareness and education which enables community members to have a better understanding of their own health issues and thus take greater responsibility for improving their own health outcomes.

About The Role
Responsibilities include coordinating and supporting the team delivering comprehensive nursing care to participants in their home.
About You
- Experience or a strong interest in one or more of the following areas:
- Palliative Care – End of Life care, symptom management and family support
- District Nursing – Wound care, medication management, chronic disease management.
- Hospital in the Home – acute care delivery in a home setting.
- Ability to contribute and practice collaboratively as part of a multidisciplinary team
- Experience working autonomously in a home-based setting with strong clinical assessment, problem solving and documentation skills.
- A compassionate and empathetic approach to patient care and the ability to work and adapt to changing participant needs.
Appointment is subject to immunisation clearance and current and satisfactory Employment Checks (Police and Working With Children Checks and NDIS Worker Screening Check).
