Home Support

Home Support is an entry-level program that helps older people to live independently in their homes and communities, preventing or delaying the need to move into residential aged care. It also provides respite services so allow carers to have time off.

The program aims to help people live as independently as possible, by providing a small amount of help to a large amount of people. The focus is to work with, rather than do things for clients, so that they can retain their independence.

Services include:

  • Meals
  • Domestic Assistance
  • Personal Care
  • Access to the community (Social Support) for individuals
  • Respite
  • Minor Home Modifications and Home Maintenance

Accessing this service

CHSP

Anyone aged over 65 years, or over 50 years for Aboriginal or Torres Strait Islander peoples, who experience limitations in their ability to live independently and need assistance can access this service.

To do so, contact ‘My Aged Care’ via this link or call 1800 200 422 within the hours 8am to 8pm on Monday – Friday or 10am to 2pm on Saturdays

HACC -PYP

We provide services to HACC-PYP eligible people (under 65. or under 50 years for Aboriginal or Torres Strait Islander peoples, for whom their capacity for independent living is at risk). This may be due to chronic illness, mental health issues, disability or other conditions based on an assessment by our HACC assessor.

Home Care Packages/Support at Home

Healthy@Home

Healthy@Home is a free phone support service assisting people to stay healthy and safe in their home.

Who can access this service?

Healthy@Home is suitable for people aged 18 years and over who may be at risk of admission to hospital. Common conditions of people in the Healthy@Home program include:

• Complex or long-term health conditions

• Reduced function and health (frailty)

• Reduced support from family and friends

• Lack of carer support

Carer Support Program

The Carer Support Program promotes physical and emotional wellbeing support to carers in our community. We understand that every caring situation is unique and the supports we offer are based on the individual needs and circumstances of each carer.

Types of services we can provide include:

  • Support groups
  • Respite
  • Advocacy support
  • Referrals to other health and community services
  • Connection to social and community activities and support
