Community Nursing – 5352 9328



Home and Community Care Program for Young People (HACCPYP) – visit clients under 65 who require support and First Nations people under 50 years old.

District Nursing Services – deliver clinical care for management and monitoring of wounds, medication and chronic disease etc.

Hospital In The Home (HITH) – deliver acute care in the home setting.

Hospital Admission Risk Program (HARP) – provide comprehensive chronic disease management and coordination with the client, other clinicians and specialists.

Post Acute Care (PAC) – support on the discharge from hospital to help transition from hospital to home.

Breast Care – Support following diagnosis and/or surgery

Hours 

  • Mon to Fri 8am – 6pm for all of Community Nursing Services.
  • Weekends and Public Holidays District Nursing and HITH only
    8am – 6pm

Other information

For referral it generally needs to come from a doctor if clinical care required.

If over 65 years old need to be registered with My Aged Care.

Visits will incur a fee of $5 and capped at $125 per month.

Help manage end of life care, symptom management and family support.

Palliative Care after hours service operates from 4.30pm until 7.15am
Mon-Thurs and 24 hours a day on weekends and public holidays for existing palliative care clients only on (03) 9209 8475.

Central Grampians Palliative Care is auspiced by East Grampians Health Service.

We provide services to Rural City of Ararat, Northern Grampians Shire and the Pyrenees Shire.
