We are seeking suitably experienced Registered Nurses to work in our Central Grampians Palliative Care team which is located at East Grampians Health Service (EGHS) Ararat.
This is a Part time position, 16 hours per week ongoing and 4 hours per week (1 shift a fortnight) limited tenure to 05.07.2026.

Community Nursing is a part of the broader Community Services Department based in Ararat. We provide community nursing across the catchment area of Ararat Rural City. Community nursing includes palliative care, district nursing, hospital in the home, hospital admission risk program (HARP), home care packages and specialist nursing programs. Each program area aims to provide awareness and education which enables community members to have a better understanding of their own health issues and thus take greater responsibility for improving their own health outcomes.

About The Role
Central Grampians Palliative Care (CGPC) is the primary provider of specialist palliative care services in the local government areas of Ararat, Pyrenees and Northern Grampians. Our team provides patient centred, evidence-based specialist services for consumers with palliative and end of life care needs, supporting consumers and their carers to feel safe and comfortable. Our team works closely with the Grampians Regional Palliative Team and is working closely with Ballarat Hospice to improve access to after-hours care and advice. EGHS also participates in the Palliative Care Outcomes Collaboration (PCOC) to benchmark and continually identify opportunities to improve our service.
About You
- Recent experience and post graduate qualifications in palliative care nursing; or willingness to work towards same
- At least 3 years’ experience in palliative care, community nursing or related area
- A demonstrated commitment to the provision care that is client centered
- A commitment to the provision of care which focuses on the individual and support networks
- The ability to contribute and practice collaboratively in a multidisciplinary team
- Demonstrate ability to assess, plan and coordinate care for clients and their carer with complex medical, psychological and chronic illness
- Written and computer skills required to fulfil this position
- A current and valid Victorian driver’s license
- Current registration with AHPRA as a Registered Nurse
Appointment is subject to immunisation clearance and current and satisfactory Employment Checks (Police and Working With Children Checks and NDIS Worker Screening Check).
