We are seeking qualified Home Care Workers to work in Community Services – Home Support Program.

East Grampians Health Service aims to meet the needs of the community by offering a vast array of services based at the community health centre in Ararat. We also offer outreach services to Willaura, Lake Bolac and Elmhurst. Our services include dental, physiotherapy, exercise physiology, occupational therapy, podiatry, speech pathology, dietetics, diabetes education and social work.  These services support other clinical areas including our acute and residential aged care patients. Our community health centre also hosts speciality clinics such as consulting suites for our visiting surgeons and maternal child and health.  

What We Offer

About The Role

You will assist clients to maintain their independence and provide ongoing domestic, personal and social support to people living in the community. In addition, you will support carers of people who are frail aged including people with dementia and people with a disability or mental health condition.

The role will incorporate travel to clients’ homes and various locations within the region involving but not limited to:

  • Personal care, showering, dressing and cooking
  • Domestic services; cleaning, grocery shopping and socialising
  • Escort to medical appointments
  • Assist with medications

About You

  • be caring and compassionate nature
  • have a high level of attention to detail
  • be committed to confidentiality and have a high level of integrity
  • hold a Certificate III in Aged Care/Home and Community Care/Individual Support/Disability
  • have a current First Aid Certificate and
  • a current and valid driver’s licence

Appointment is subject to immunisation clearance and current and satisfactory Employment Checks (Police and Working With Children Checks and NDIS Worker Screening Check).
