We are looking for a team member with strong administration and communication skills to join our Healthy@Home department in the role of Telehealth Assistant.

East Grampians Health Service (EGHS) Community Services aim to meet the needs of the community by offering a vast array of services to the community in Ararat and surrounding districts. We offer outpatient, outreach and domiciliary services. Our services include Allied Health, Dental, Community Nursing, Home Care Packages, Patricia Hinchey Centre and EGHS@Home.  These services support other clinical areas including our acute and residential aged care consumers. Our Community Health Centre also accommodates Maternal Child and Health Services and consultation rooms for visiting medical, surgical and clinical services.

What We Offer

About The Role

The Healthy@Home Program aims to keep our most vulnerable clients, especially those with complex health issues, healthier at home, aiming to reduce avoidable hospitalisations and presentations to our Urgent Care Centre. Healthy@Home uses regular phone calls to clients to conduct self-rated health checks that are designed to identify deteriorating health.

The Telehealth Assistant’s role is to provide regular phone calls to clients, asking specific questions which allows the client to rate their health. The Telehealth Assistant enters the client’s responses in real time into computer software, which generates flags when a client’s health is potentially deteriorating. This allows for the Care Co-ordinator to review the call and arrange an appropriate response. The Telehealth Assistant will work closely with Care Co-ordinators to ensure their clients get the help they need in a timely manner. Through regular phone calls, the Telehealth Assistant provides a consistent point of contact for the client and/or their carer.

Appointment is subject to immunisation clearance and current and satisfactory Employment Checks (Police and Working With Children Checks and NDIS Worker Screening Check).
