Carer Support Program Coordinator – EGHS@Home


We are seeking a Carer Support Program Coordinator to work in Community Services – EGHS @ Home. This is a part time position, 24 hours per week.

East Grampians Health Service (EGHS) Community Services aim to meet the needs of the community by offering a vast array of services to the community in Ararat and surrounding districts. We offer outpatient, outreach and domiciliary services. Our services include Allied Health, Dental, Community Nursing, Home Care Packages, Patricia Hinchey Centre and Home Care Services.  These services support other clinical areas including our acute and residential aged care consumers. Our Community Health Centre also accommodates Maternal Child and Health Services and consultation rooms for visiting medical, surgical and clinical services.

What We Offer

About The Role

You will be responsible for identifying carers needs and responding to those needs by offering appropriate services and facilitating informal group meetings. You will need to be advocate for the recognition and needs of carers. You will have the ability to promote the Carer Support Program and follow referral pathways to expand the program. You will be responsible for reporting against funding targets.

About You

Our ideal candidates will:

  • Be caring and compassionate in nature
  • Be able to facilitate group meetings with strong communication skills
  • Have a high level of attention to detail
  • Ability to work independently and have highly developed organisation and time management skills
  • Be committed to confidentiality and have a high level of integrity
  • Have a current First Aid Certificate and
  • Hold a current and valid driver’s licence

Appointment is subject to immunisation clearance and current and satisfactory Employment Checks (Police and Working With Children Checks and NDIS Worker Screening Check).
