PALLIATIVE CARE CLINICAL COORDINATOR

If you are an experienced Palliative Care clinician, this is your opportunity to take the next step in your career, providing clinical support to a small team and growing our capacity to respond to community need.  This role reports to and is supported by the Community Nursing Manager.

We are seeking a Registered Nurse – Grade 4 for 4 days per week (0.8/32 hours) – Negotiable.


East Grampians Health Service (EGHS) is the primary provider palliative care services in the local government areas of Ararat, Pyrenees and Northern Grampians.   Our team provides patient centred, evidence based specialist services for consumers with palliative and end of life care needs, supporting consumers and their carers to feel safe and comfortable.   The service collaborates with the Grampians Palliative Care Consortium and is working with Ballarat Hospice to improve access to after-hours care and advice.  EGHS also participates in the Palliative Care Outcomes Collaboration (PCOC) to benchmark and continually identify opportunities to improve our service.
