Hospital Admissions Risk Program (HARP) Care Coordinator

An opportunity exists for a HARP Care Coordinator in our Community Nursing team based at East Grampians Health Service, Ararat.  This is a part time position 20 hours per week.

Community Nursing is a part of the broader Community Services Department which is located at the Ararat campus, we provide community nursing across the catchment area of Ararat Rural City. Community Nursing includes Palliative Care, District Nursing Hospital in the Home, Hospital Admission Risk Program (HARP), Home Care Packages and specialist nursing programs. Each program area aims to provide awareness and education which enables community members to have a better understanding of their own health issues and thus take greater responsibility for improving their own health outcomes.

About the Role

The Hospital Admissions Risk Program (HARP) Care Coordinator will facilitate the provision of care to clients with chronic and complex illnesses within the Ararat Rural City catchment, identifying clients that are eligible for the program and providing comprehensive assessment, support, care management and coordination of services to ensure enhanced care and coordination to reduce the risk of hospitalisation.

About You

Appointment is subject to immunisation clearance (Including COVID-19) and current and satisfactory Employment Checks (Police and Working With Children Checks).
